Polio vaccination programs not reaching enough children in Afghanistan and Pakistan

Polio vaccination programs not reaching enough children in Afghanistan and Pakistan

Wednesday, July 4, 2012

New, more effective vaccines are struggling to have an impact in the drive to eradicate polio in Afghanistan and Pakistan because not enough children are being vaccinated, according to a study published in the Lancet medical journal today. Sharp declines in vaccine uptake led to a rise in the number of new infections between 2006 and 2011, even though new vaccines introduced during this time have proven to be more effective against the main circulating strain of the virus.

Poliovirus exists in three strains, with type 1 the most prevalent and type 2 unseen anywhere in the world since 1999. It mainly affects children under five and causes paralysis in about one in 200 cases. In most parts of the world it has been eliminated by sustained vaccination programmes in the late 20th century. Afghanistan and Pakistan are among only three countries ? along with Nigeria ? that have never managed to eliminate polio.

Since it was launched in 1988, the Global Polio Eradication Initiative (GPEI) ? spearheaded by national governments, the World Health Organization (WHO), Rotary International, the US Centers for Disease Control and Prevention (CDC) and UNICEF - has relied on mass campaigns using the trivalent oral poliovirus vaccine, which contains all three types of the virus,. However, the presence of the type 2 strain in the trivalent vaccine makes it less effective against the other types. In 2005, monovalent vaccines of types 1 and 3 were introduced by the GPEI and in 2009 a bivalent vaccine containing both types was first used.

Scientists from Imperial College London and the WHO assessed the effect of immunisation campaigns in the Afghanistan and Pakistan by analysing data collected by national surveillance programmes. They also assessed the effectiveness of the three varieties of polio vaccine against the predominant type 1 strain of the virus.

The study found that the monovalent vaccine was more effective than the trivalent vaccine against type 1 poliovirus. The bivalent vaccine was comparable in effectiveness to the monovalent vaccine. The difference between the bivalent and trivalent vaccines was not large enough to be statistically significant.

In 2006 over 80% of children in Afghanistan and Pakistan aged 0-2 years old received four doses or more of vaccine. However, vaccine coverage in southern Afghanistan and parts of Pakistan, including Balochistan and the Federally Administered Tribal Areas (FATA) declined from 2006 to 2011. Although the impact was partially offset by the use of the more effective vaccines, this resulted in significantly lower population immunity levels, leading to an increase in cases over that time frame. In 2011 there were 198 cases in Pakistan and 80 cases in Afghanistan compared with 40 cases in Pakistan and 31 in Afghanistan in 2006.

This analysis prompted a major shift in approaches in 2012 in both countries, to address the underlying challenges which prevent children from having access to polio vaccines. Both countries are this year implementing national polio emergency action plans, involving measures such as increasing technical support to the worst-performing areas; creating accountability mechanisms for district-level leadership; and engaging with communities to increase demand for immunisation.

"Our findings show it's not just small pockets where vaccine coverage decreased from 2006 to 2011," said Dr Kath O'Reilly, from the Medical Research Council (MRC) Centre for Outbreak Analysis and Modelling at Imperial College London, who led the study. "The new vaccines appear to be more effective than the trivalent vaccine, which offers encouragement that polio eradication is achievable. But the best vaccine in the world will not work unless it reaches the children its intended to protect. So it's vitally important that vaccine coverage is considerably improved in 2012 through better campaigns and routine programmes if we're to finally eradicate polio globally."

Mexico's Pena Nieto to push for quick reforms

MEXICO CITY (Reuters) - Mexican President-elect Enrique Pena Nieto pledged on Monday to focus on energy, labor and tax reforms and said he hopes to strike deals with opponents to help shepherd changes through Congress before he takes office in December.

Pena Nieto won Sunday's election with about 38 percent of the vote, good for a lead of about 6 percentage points over his nearest rival, returning his Institutional Revolutionary Party (PRI) to power after 12 years in opposition.

But the victory margin was smaller than expected and results suggested the PRI and its Green allies would struggle to win a majority, officials at the electoral authorities told Reuters.

That would leave Pena Nieto reliant on other parties to back his plans to reinvigorate Latin America's No. 2 economy.

Speaking to reporters in Mexico City, the 45-year-old said he was ready to consult with outgoing President Felipe Calderon and bring in policy experts to get work done on the economic reforms and help ease their path through Congress, which convenes in September.

His main reform proposals include allowing more private investment in Mexico's state-run oil industry, overhauling the tax system to improve government revenues and liberalizing the country's labor laws to encourage job creation.

"We will now be working on all these initiatives with public policy experts," Pena Nieto told a news conference. "These are the initiatives that will enable us to give shape to the structural reforms I have promised."

Calderon's conservative National Action Party (PAN) had tried to get similar reforms through Congress over the past six years but the efforts were thwarted by opposition from the PRI, which has both populist and pro-business tendencies.

Pena Nieto has promised to lift economic growth to about 6 percent a year, create jobs and draw the heat out of a war with drug gangs that bogged down Calderon's administration. The conflict has killed more than 55,000 people since late 2006.

Long regarded as corrupt and authoritarian, the PRI has bounced back under the youthful Pena Nieto, who has vowed to break with the party's checkered past.

He has sought to bring in new blood to the party, and Pena Nieto said his campaign chief, Luis Videgaray, 43, would form part of his government team. Videgaray is well regarded by investors and seen as a possible choice for finance minister.

But the party still faces deep resentment from many Mexicans.

"Yes, the PRI has experience. They know how to steal. They know how to make pacts with drug cartels. And they know how to kill," said Heliodoro Maciel, an electrician and trade unionist.

Opinion polls in the last days before the election had shown Pena Nieto winning by between 10 and 15 percentage points, but with 97 percent of returns in, the gap with his leftist rival, Andres Manuel Lopez Obrador, was 6.4 points, or roughly 3 million votes.

PAN candidate Josefina Vazquez Mota came in a distant third as the PAN suffered a crushing defeat, hurt by Calderon's failure to ramp up growth and curb the drug war violence.

The PAN raised high hopes when it was elected in 2000, but the economy underperformed its peers in Latin America for most of the 12-year rule by the party, which never had a majority in Congress and was unable to push through reforms.

"Nothing has improved since the PAN got in," said Mexico City plumber Raimundo Salazar, 44. "The PRI understands how things work here. And it knows how to manage the drug gangs."

FINANCES UNDER STRAIN

Lopez Obrador said on Sunday night it was too early to concede defeat, but Calderon and U.S. President Barack Obama have already congratulated Pena Nieto on his triumph.

The U.S. State Department said it expected close cooperation against organized crime to continue under Pena Nieto.

Pena Nieto will take over at a time when Mexico's finances are in good order and the economy is improving, although it still cannot generate enough jobs for the growing population.

The election result helped bolster Mexico's main share index early on Monday before weak global manufacturing data hit stocks and the peso currency.

Lopez Obrador could still choose to challenge the election, as he did six years ago when he narrowly lost to Calderon and launched months of protests, alleging fraud.

He has said in recent weeks this election campaign was also plagued with irregularities, raising concerns he might again call his supporters onto the streets. On Sunday night, he said only that he would wait until all the results were in.

Google shocker: American web company says web boosted American economy by $500 billion

Google bronze sign

Who knew that a company which makes a lot of money on the web would be keen to pitch the web as important to the US economy? Google thinks it's as natural as breathing, as it just took significant credit for fueling real-world shopping. About 97 percent of Americans reportedly searched online for a local business -- it's safe to say Google was involved in a lot of those hunts -- and spent an average of nearly $2,000 per person at brick-and-mortar stores last year. The search giant extrapolates to argue that the web pumped about $500 billion into the US economy, even while excluding internet-only sales. That's certainly a staggering number that shows how much of a cornerstone the internet has become for American business, although it comes across as slightly insecure: a company that's reportedly under tight scrutiny from regulators might be eager to show just how much good it's doing for the public in tough economic times.

Why Apple would release a 7 inch iPad

Back in May, iMore heard that Apple planned to move ahead with a 7-inch iPad this fall at a $200 price point. Stories about a smaller iPad have been around for a while, but as far as I know that was the first time such aggressive pricing got tagged onto them. The reasons Apple was planning this, we heard, was the same reason they planned and executed on the lower price point iPod mini and iPod nano -- to take the oxygen out of the market. In this case, to leave no room for discount competitors like Amazon and Google. Some have a hard time believing Apple would enter the low end tablet market. Apple didn't make a netbook and didn't release an iPhone nano. They thought differently and released the MacBook Air and the iPad, and kept around previous generation iPhones instead. Steve Jobs went so far as to say 7 inch tablets were DOA. So Apple couldn't, wouldn't, and shouldn't do an iPad mini, right?

Is health care ruling Obama's 'Read my lips: No new taxes' moment?

Republicans are beating up on President Obama because the Supreme Court said the Affordable Care Act is tied to new taxes. But that could be a problem for Mitt Romney given his record in Massachusetts.

Remember ?Read my lips: No new taxes? ? George H. W. Bush?s infamous pledge in 1988, which he was forced to break as president and which Bill Clinton used to hammer Mr. Bush four years later?

Republicans certainly do, and they?re using it target President Obama ? both on the specifics of the Affordable Care Act, which they vow to repeal, and to paint Obama as a tax-and-spend liberal.

Here?s how we got to this moment:

In launching what he?s now happy to call ?Obamacare? back in 2009, the President likened the proposed penalty for not having health care coverage to car insurance. "Nobody considers that a tax increase,? he told ABC News. ?People say to themselves, 'that is a fair way to make sure that if you hit my car, that I'm not covering all the costs.' "

It was a point repeatedly made by the White House and congressional Democrats as the ACA fought its way to passage. No new taxes.

How much do you know about health-care reform? Take our quiz!

Fast-forward to last week and the US Supreme Court?s startling decision to uphold the ACA as constitutional.

Speaking for the court majority regarding the individual mandate, Chief Justice John Roberts wrote: "The Affordable Care Act's requirement that certain individuals pay a financial penalty for not obtaining health insurance may reasonably be characterized as a tax."?

Boom. Obamacare passes constitutional muster because its chief element is a tax.

"The Supreme Court has spoken,? declared Senate Republican Leader Mitch McConnell. ?This law is a tax."

Americans for Prosperity, the conservative political action committee funded by the Koch brothers, has just launched a $9 million ad campaign targeting the ACA and running in 12 swing states.

"Obama's health care law is actually one of the largest tax increases in history," the ad's narrator says. "Shouldn't Obama's priorities have been creating jobs and ending reckless spending?"

Meanwhile, key Democrats were all over the Sunday morning TV blabfests denying the charge.

Rangers, Rockies on historic paces

With two teams already there and the rest set to hit the halfway point of the season within the next few days, I wanted to look at what kind of pace each team was on. Below are the number of wins each team would finish with given its current pace, plus how that would stack up against the team?s history. As things stand now, there?s one team on pace for its most wins ever and another on pace for its fewest.

AL East
Yankees ? 100 ? most since 2009
Orioles ? 87 ? most since 1997
Red Sox ? 86 ?- fewest since 2001
Rays ? 84 ? fewest since 2009
Blue Jays ? 82 ? most since 2010

AL Central
White Sox ? 86 ? most since 2010
Indians ? 83 ? most since 2007
Tigers ? 80 ? fewest since 2008
Royals ? 74 ? most since 2008
Twins ? 69 ? most since 2010

AL West
Rangers ? 101 ? most ever
Angels ? 90 ? most since 2009
Athletics ? 77 ? most since 2010
Mariners ? 68 ? most since 2009

NL East
Nationals ? 95 ? most since 1979*
Mets ? 87 ? most since 2008

Marlins ? 79 ? most since 2010
Phillies ? 72 ? fewest since 2000

NL Central

Pirates ? 87 ? most since 1992
Cardinals ? 84 ? fewest since 2007
Brewers ? 75 ? fewest since 2006
Astros ? 66 ? most since 2010
Cubs ? 60 ? fewest since 1981**

NL West
Giants ? 91 ? most since 2010
Dodgers ? 89 ? most since 2009
Diamondbacks ? 81 ? fewest since 2010
Rockies ? 62 ? fewest ever
Padres ? 61 fewest since 1993

*The Expos were 74-40 and on pace to win 105 games when the strike ended the 1994 season.

**The 1981 Cubs went 38-65 in that strike-shortened season, which also works out to 60-win pace. Their last full season under 60 wins was 1966, when they went 59-103.

If the Rangers keep at it, they?ll set a franchise high for victories for the second straight year, topping last year?s total of 96. Before that, their high was 95 win sin 1999.

The Rockies are on pace for their fewest wins, excluding the 1994 strike season (they were on pace for 73 wins that year). Their low total is 67 wins, from both their inaugural season in 1993 and from 2005.

The Yankees and Rangers are both on pace for 100 wins. Only one team, the Phillies, got there last year. No team did in 2010. If the Yankees can do it, it?d be their sixth 100-win season since 1998.

And then there?s the Pirates, on pace for their first .500 season since Barry Bonds left. Their high total since was 79 wins in 1997.
